Standard LASIK would not be suitable for you due to the large pupils, you would need to have the Wavefront addition and even then there are risks that night vision could still become impaired.  These are all things that we would test for and discuss during the consultation.  If the pupils are too large, we would either consider LASEK or not proceed with the treatment for you.

The price of treatment increases with the strength of the prescription so 4 dioptres you would be looking at round about £995 (this is estimate based on today's prices) per eye.  However, based on the information you would not be suitable for standard treatment.  You would need at least LASIK Wavefront or LASEK Wavefront as a minimum.  We can give you the exact cost at consultation.
